1. Personal appearance of applicant
2. Accomplished application form
3. Passport valid for at least six (6) months beyond the intended period of stay in the Philippines (original plus 2 photocopies of datapage)
4. 1 Recent passport-size colored photo
5. Confirmed airline booking (2 copies)
6. Copy of Seaman’s book
